Advanced or Honors Diploma Application |
In addition to graduation requirements, all Utah public high school students who plan to graduate on or after January 1, 2016 must pass a basic civics test in order to receive a diploma. Click here to find out more.

- Navigate to http://canyons.instructure.com and sign in with your student account.
- If you are prompted to accept an invitation to the CCHS Civics Test Course, click Accept and continue to Step 5.
- On the left sidebar navigation, click Courses, then All Courses.
- Click the CCHS Civics Test course.
- When the course loads, click the Take the Basic Civics Test link.
- Click the blue Take the Quiz button. Take the quiz and submit when finished. You need to receive a score of 70% or better to complete this graduation requirement. If you score less than 70%, you may retake the test until the necessary score is obtained
Are you Bilingual? Apply for a Special Seal on your Transcripts

Calling all bilingual 11th and 12th grade students: If you’re fluent in two or more languages, you can apply to have an official Seal of Biliteracy added to your high school transcripts.
The Seal of Biliteracy is placed on a high school graduate's transcript by the state of Utah to certify for employers and universities that the student has demonstrated proficiency in English and at least one world language. It is evidence of a student's readiness for a career, college and for engagement as a global citizen.
High school juniors and seniors are eligible to apply for the seal starting Tuesday, December 1, 2022. The application window closes on Friday, January 20, 2023. The testing date has not been posted but will be held between March 1st-17th. There are no fees associated with the first application. But if a student chooses to demonstrate fluency in more than one world language, a $20 testing fee may apply.
To apply, students must fill out the Seal of Bi-Literacy Application on-line (login with your CSD login/password). As part of the application process, students may be required to take a language proficiency exam sometime between March 1-12. Individual schools will determine the date, time and location of testing as well as a make-up testing date. For frequently asked questions, click here.

Charger MedallionTo be honored with the Charger Medallion at the 2023 CCHS graduation, graduating seniors, over the course of their time at CCHS, must have demonstrated an exceptionally high level of school involvement. Students are responsible for completing & submitting the application online prior to Friday, April 28th, 2023. For detailed information and to access the application, click here.
